The prospect can just use one of these, and the other 3 are
for their relatives (friends, family, colleagues …). The format
of the pass will be cardboard (I can create them).
To pratice, they just have to leave their contact information on
the pass, and to give us. We could then use it for the
newsletter.

By this way, each prospect will talk about your gym to their
loved ones, which will increase the number of prospects.
